Request Training

  • Visit REPORTER to register for a course.
  • University’s Fire and Life Safety staff is available to conduct onsite training, specific to your group’s interest. To schedule a course for your department, office, or organization, please contact the Fire & Life Safety Training program coordinator.
    • Please note: individual class requests need at least 10 people before scheduling.

Request Extinguisher

Other Forms

Temporary structure permits are required for events and instances where tents, stages and other membrane structures will be in use for a limited period of time. The following requirements apply for Tents, Canopies, and Membrane structures with sides over 700 square feet or without sides over 1800 square feet, 75% to 100% open sides.

Hot work permits are required for any welding, cutting, torching, brazing, soldering and other related activities. This applies to all contractors and University units conducting hot work on campus.

Fire & Life Safety Consult Requests are for students, staff and faculty to provide information and receive consultation directly from FLS.

A Recreational Fire Permit is required for all outdoor cooking, grilling, frying or open flame activity taking place on University property. Stationary grills located outside residence halls are exempt and do not require a permit to use. A Recreational Fire Permit is also required before use of a personal grill, DASA grill, or Fire Pit on University property.

The AED Acquisition Form is used to request and document new AED units. The AED Policy & Procedure outlines the guidelines and operational protocols for using the AEDs. The AED Monthly Inspection Form ensures regular maintenance and checks are conducted on each device. The AED Use Report Form records details of any incidents where the AED was used.
